I think the Catholic church has the best model each church has its own local board and local group of volunteers. And than a head priest. Also the richer parish's help fund the poorer ones. Usually individual church's have their own charity (a soup kitchen, letting local aa groups use their space. But than their is a larger structure which has its own Charities...
